Choice of diuretics in cirrhosis with ascites.

چکیده
Thirty non azotemic cirrhotics with ascites were treated with oral diuretics on outpatient basis. Estimation of 24 hrs urinary sodium levels proved useful in the diagnosis of secondary hyperaldosteronism (63.3% cases) and the determination of type and dose of diuretics required. Prompt and effective clearance of ascites occurred within 4 weeks of the therapy. No adverse effects were noted and no hospitalisation was required. The regimen was found to be cost effective for developing countries (JPMA : 37 : 142, 1987).

Pharmacological Therapy of Ascites
Ascites refer to accumulation of fluids in the peritoneal cavity. Ascites is caused by multiple causes, among which liver cirrhosis is the commonest. Confirming the etiology is the first and most important step toward proper management. Assuming that ascites is always caused by cirrhosis can lead to unnecessarily sending patients with different etiologies for liver transplantation, particularly...
متن کاملClinical Use of Diuretics in Heart Failure, Cirrhosis, and Nephrotic Syndrome
Diuretics play significant role in pharmacology and treatment options in medicine. This paper aims to review and evaluate the clinical use of diuretics in conditions that lead to fluid overload in the body such as cardiac failure, cirrhosis, and nephrotic syndrome. To know the principles of treatment it is essential to understand the underlying pathophysiological mechanisms that cause the need ...

متن کاملManagement of adult patients with ascites caused by cirrhosis.
Ascites is the most common of the major complications of cirrhosis. The development of ascites is an important landmark in the natural history of cirrhosis and has been proposed as an indication for liver transplantation. The initial evaluation of a patient with ascites should include a history, physical evaluation, and abdominal paracentesis with ascitic fluid analysis. Treatment should consis...
متن کاملTIPS and refractory ascites. Lessons from the recent history of ascites therapy.
EFRACTORY ascites, a term introduced during the R 1950’s to define the ascites not responding to sodium restriction and diuretics, was an extremely frequent condition prior to the 1960’s, when the medical treatment of ascites consisted of the administration of mercurial or thiazide diuretics. These agents have a poor natriuretic effect in patients with marked hyperaldosteronism, who represent a...
